Sinusitis or just a bad cold? How to tell the difference (and when to see a doctor)

Congestion, facial pressure, a headache that won't quit: it's easy to assume you're just dealing with a lingering cold. But if symptoms are sticking around longer than expected or getting worse instead of better, it might be a sinus infection.

Telling the two apart matters, because the right next step depends on which one you're actually dealing with.

How a cold usually behaves

A common cold typically peaks within a few days and starts improving on its own within seven to ten days. Symptoms tend to include a runny or stuffy nose, mild congestion, sneezing, a scratchy throat, and maybe some low-grade tiredness, all of which gradually fade rather than get worse.

Signs it might be a sinus infection

Sinusitis often starts out looking like a cold but tends to stick around longer or take a turn for the worse. Watch for:

  • Facial pain or pressure, especially around the cheeks, eyes, or forehead
  • Thick, discolored nasal discharge
  • A cold that seems to improve and then suddenly gets worse again
  • Symptoms lasting more than 10 days without improvement
  • A headache that's worse when you bend forward
  • Reduced sense of smell
  • Bad breath not explained by anything else

When it's worth getting checked out

Most sinus infections, viral or bacterial, can be managed without an emergency visit. Book a visit if you notice:

  • Fever above 102°F
  • Severe headache or facial pain that isn't improving
  • Symptoms lasting more than 10 days, or improving and then worsening again

When to get emergency care

Call 911 or go to the nearest ER if you have:

  • Swelling or redness around your eyes
  • Vision changes
  • Confusion
  • A stiff neck
  • A severe headache with high fever

These can be signs the infection has spread and needs to be seen in person right away.

Sinus infection treatment online

A video visit is often enough to tell the difference between a cold running its course and a sinus infection that needs treatment. Your provider reviews your symptoms and timeline, decides whether antibiotics are appropriate or supportive care is the better call, and sends an online prescription to your pharmacy if you need one. No in-person visit is required for most straightforward cases.

At SofaCare, visits are witha Florida-licensed physician or physician assistant, for a flat $49 self-pay price with no insurance needed, Monday through Friday, 9am–5pm ET, for adults 18+ anywhere in Florida.
